A new medication for attention-deficit hyperactivity disorder has entered the medical landscape. Known as centanafadine and marketed as Simtriyo, this daily tablet is now approved in the United States for both adults and children aged six and older.

What distinguishes this drug from previous treatment options is its chemical action. Most current ADHD medications target two brain chemicals: dopamine and noradrenaline. Centanafadine is the first in its class to affect these two chemicals while also targeting serotonin. By slowing the reuptake process, the drug allows these neurotransmitters to remain active in the brain for longer periods.

While stimulants like methylphenidate and lisdexamfetamine primarily manage dopamine and noradrenaline, the addition of serotonin makes this drug a unique option for clinicians. Debate continues regarding its classification as a stimulant, as the manufacturer labels it a non-stimulant while the FDA includes it under stimulant documentation. Regardless of terminology, its ability to influence three pathways offers a potential alternative for patients who do not respond to traditional options.

Four phase-3 clinical trials supported the approval of this medication, showing improvement in ADHD symptoms compared to a placebo. Participants reported better self-control and attention, with results appearing in teenagers within one week. Common side effects align with existing ADHD treatments, including reduced appetite, insomnia, and headaches. Researchers also noted a standard warning regarding suicidal thoughts, which remains a requirement for this class of medication.

At present, the drug is not available in Australia, as it awaits review from the Therapeutic Goods Administration. Furthermore, clinicians remain cautious about direct effectiveness comparisons. Because current studies compared the drug to a placebo rather than other existing medications, scientists cannot yet confirm if it outperforms currently available treatments. Ongoing monitoring will determine its long-term role in managing ADHD, particularly for patients who may also experience symptoms of anxiety or depression.
