Data Links Weight-Loss Drugs to Vision Risk

A large medical review suggests patients using semaglutide for diabetes or weight management may face higher risks of a rare eye condition. The findings appeared in the journal Neurology and highlight a potential link between the drug and nonarteritic anterior ischemic optic neuropathy. This condition restricts blood flow to the optic nerve, leading to sudden, painless vision loss. While the absolute risk remains low, the study points to a 152 percent higher relative risk for semaglutide users compared to those on other treatments.

Nonarteritic anterior ischemic optic neuropathy remains the primary cause of acute optic nerve injury in older populations. It occurs when blood flow to the nerve is restricted, causing tissue to swell. Traditional risk factors include high blood pressure, sleep apnea, and type 2 diabetes. Because semaglutide users often manage these exact conditions, isolating the drug as a cause poses a challenge for clinical researchers.

The Scale of the Investigation

Researchers from the University of Western Ontario and the University of Toronto performed a meta-analysis of observational studies published before April 2025. They pooled data from five major studies, representing a combined cohort of 1,593,554 patients. Of this group, 682,456 patients used semaglutide, while 911,098 used alternative medications. The team used Bayesian statistical models to account for the rarity of the eye condition.

Patients with diabetes showed a 141 percent higher risk when taking semaglutide. In the obesity subgroup, the relative risk reached 77 percent. The study calculated an incidence of 118 cases per 100,000 semaglutide users. When compared to SGLT2 inhibitors, semaglutide users experienced double the risk of developing the condition. The researchers noted that statistical certainty varied by subgroup but remained strong across the aggregate data.

Scientific Hypotheses and Clinical Context

The biological mechanism remains unproven. One theory suggests the medication's impact on sodium absorption and blood pressure might reduce local blood flow to the optic nerve. Rapid weight loss may also contribute to lower blood pressure, potentially leaving the eye vulnerable if patients do not adjust other blood pressure treatments. Still, the observational nature of the data prevents definitive causal claims.

These drugs offer significant systemic benefits, including reduced cardiovascular death and heart attack risk. Experts suggest the overall health improvements for most patients likely outweigh the low absolute risk of optic neuropathy. Future prospective trials are necessary to confirm if these medications directly influence ocular blood flow. Doctors should maintain discussions with patients about potential visual changes as part of the prescribing process. Any patient experiencing sudden vision shifts should seek immediate ophthalmic evaluation to prevent permanent damage.
