Investigation Into Prenatal Drug Exposure and Reproductive Development

Researchers in Copenhagen tracked 302 infants to determine whether prenatal exposure to acetaminophen, commonly sold as Tylenol or generic paracetamol, affects female reproductive organs. The findings, published Wednesday in Human Reproduction Open, show that 3-month-old girls exposed to the drug in utero possessed smaller ovaries, fewer ovarian follicles, smaller uteruses, and lower levels of specific reproductive hormones compared to infants not exposed to the medication.

Margit Bistrup Fischer of Rigshospitalet led the team, which relied on questionnaires and urine samples to track drug intake among mothers. Exposure occurred within recommended daily limits, as none of the participants exceeded 4,000 milligrams. Most mothers used the medication to manage common complaints like headaches or musculoskeletal pain. Despite the observed differences in measurements, Fischer stated that the study does not establish a causal link between the drug and these physiological changes.

Data Analysis and Long-Term Implications

Specific measurements highlight the scale of the findings. Infants exposed during fetal development showed a 40% reduction in ovarian volume, a 13% reduction in uterine volume, and 23% fewer ovarian follicles. Those exposed before 17 weeks of gestation also displayed lower levels of Anti-Müllerian hormone, a key indicator of egg quality and quantity. The researchers also examined a separate cohort of 1,210 girls followed from infancy through adolescence. This longitudinal look indicated that prenatal exposure correlated with smaller uterine sizes at puberty and smaller ovaries during the adolescent years.

Understanding the importance of these findings requires context about human biology. A female’s reproductive window is established before birth, as girls are born with their total lifetime supply of eggs. While animal studies previously suggested that fetal exposure to this medicine might impact ovarian egg reserves, this report provides the first such data in human subjects. Still, the authors caution that individual outcomes remain unpredictable. Many daughters of women who used the drug during pregnancy showed reproductive measurements within the normal range.

Official Guidance and Industry Response

Kenvue, the company that owns the Tylenol brand, issued a response regarding the methodology. The company noted that the research does not support a direct causal relationship between acetaminophen and postnatal development. Kenvue advises pregnant or breastfeeding individuals to consult with a healthcare professional before taking the medication, which the company maintains is the most scrutinized pain reliever for use during pregnancy. Following the release of the study, company shares dipped by more than 3%.

Medical experts offer a split perspective on how to interpret these results. Dr. Christian De Geyter of the University Hospital of Basel noted that the findings align with existing animal research. He suggested that clinical recommendations regarding prenatal use of this drug warrant further scrutiny. Currently, health authorities continue to list acetaminophen as a safe option for managing pain and fever. Untreated conditions like high fever pose documented risks to both mother and fetus, and Fischer emphasized that women should not feel alarm based on these specific results. Future research must track these cohorts through menopause to understand whether these early developmental differences translate into clinical fertility issues.
