Recent findings from Yale University provide significant insights into the biological origins of two often misunderstood conditions: trichotillomania, which involves repeated hair-pulling, and excoriation disorder, characterized by chronic skin-picking. These conditions belong to a category known as body-focused repetitive behaviors, or BFRBs. Estimates suggest these disorders affect between 1% and 3% of the general population. While they have historically been dismissed as mere bad habits, they frequently cause intense distress and physical pain, while disrupting daily functions at school or the workplace.
Emily Olfson, MD, PhD, led the investigation at the Yale Child Study Center. The work aims to replace speculation about the causes of BFRBs with concrete genetic data. Researchers specifically targeted the potential overlap between these behaviors and obsessive-compulsive disorder, given that the two are frequently classified together due to shared symptom profiles. The results appeared in the June 2026 issue of Translational Psychiatry, marking a move toward a more rigorous understanding of the underlying biology.
Methodology and the Use of Family-Based Data
The research team, which included Samantha Greenspun and Thomas Fernandez, MD, employed a family-based approach. They collected DNA from over 100 parent-child trios. This method requires samples from an affected individual and both biological parents to determine which genetic markers are passed down more frequently than statistical chance would predict. Recruiting participants involved partnerships with organizations like The TLC Foundation for BFRB, along with direct outreach to clinics.
This specific study design allows researchers to isolate inherited traits from environmental factors. By focusing on trios, the investigators could pinpoint whether affected children inherited specific risk variants from their parents that non-affected siblings did not. The scale of the recruitment highlights the desire among affected families to better understand these conditions, which have long lacked targeted therapeutic interventions.
Uncovering Genetic Risk and Biological Patterns
The study identified that individuals with BFRBs frequently inherit a higher polygenic score related to OCD. This score represents the cumulative effect of many common genetic variants, each contributing a small amount to overall risk. Crucially, this pattern appeared in participants who did not meet the criteria for an OCD diagnosis, suggesting that these conditions share a biological foundation that exists independently of a formal OCD label.
Beyond these common variants, the researchers found rare copy number variants in some participants. These represent DNA segments that are either missing or duplicated. Several of these identified genes play roles in how neurons build and organize connections within the brain. This observation links BFRBs to the broader field of neurodevelopmental research, indicating that the way brain cells connect may be a key factor in these repetitive behaviors.
Clinical Implications and Future Directions
For families struggling with these behaviors, the findings offer a path away from self-blame. The research confirms that trichotillomania and excoriation disorder stem from complex biological interactions rather than parenting styles or a lack of willpower. It is vital to note that these genetic markers are not a blueprint for behavior.
