New research published in Translational Psychiatry links hair-pulling and skin-picking behaviors to an inherited genetic risk profile shared with obsessive-compulsive disorder. These actions, known as body-focused repetitive behaviors, affect between 1% and 3% of the population. The findings confirm that these conditions stem from biological factors rather than behavioral choices or parenting styles.
The Scope of the Yale Study
Emily Olfson, an assistant professor at the Yale Child Study Center, led the research team in this parent-child trio genomic study. This approach involved collecting DNA from affected individuals and both of their biological parents. By comparing genetic markers, the team tracked how specific risk factors passed from one generation to the next. More than 100 families participated in this project, with recruitment facilitated by The TLC Foundation for Body-Focused Repetitive Behaviors.
These families played a central role in providing the data required to look at the genetics of these conditions. The research team noted that their direct engagement with these families highlighted the urgent need for better clinical understandings. Many of these behaviors remain difficult to treat in standard medical settings. Establishing a biological basis provides a foundation for more effective future interventions.
Genetic Overlap with Obsessive-Compulsive Disorder
Data analysis revealed that individuals with trichotillomania, or repetitive hair-pulling, and excoriation disorder, known as skin-picking, often inherit a higher polygenic risk score for obsessive-compulsive disorder. This score is not a single gene responsible for the behavior. It represents a calculation of many common genetic differences that contribute to risk. The study found this inherited risk exists even in participants who do not meet the criteria for a secondary OCD diagnosis.
Beyond common genetic variations, the researchers identified rare copy number variants in some participants. These variants involve missing or duplicated stretches of DNA. Similar structural DNA changes appear in various neurodevelopmental conditions. This suggests that hair-pulling and skin-picking share underlying neurological pathways with other recognized brain-based disorders.
Moving Toward Evidence-Based Care
These results provide a clear message for clinicians and patients. The researchers emphasize that these behaviors are not rooted in a lack of self-control or attention-seeking. They are legitimate medical conditions that require specialized care. This research effectively debunks historical myths that blamed parents or the environment for these habits.
Looking ahead, the team plans to expand these findings using larger groups of participants. Further genomic studies will refine the understanding of how these disorders manifest at the molecular level. This work serves as a necessary step in developing targeted, evidence-based treatments for patients who have historically lacked access to effective help.
