A recent study in Science Immunology sheds light on why patients respond differently to CAR-T cell therapy. Researchers examined whole-genome sequencing data from over 200 lymphoma patients who received axicabtagene ciloleucel across the ZUMA-1 and ZUMA-7 clinical trials.
The findings indicate that germline genetic variants are linked to both how well the therapy performs and the risk of severe side effects. Specifically, variations in the PTPN22 gene show a strong connection to increased CAR-T cell expansion, which remains a primary indicator of clinical success. Conversely, variants in the ADAMTSL3 gene appear to offer protection against treatment-related toxicities in both trials.
The analysis also identified links between STXBP2 variants and inflammatory toxicity observed in the ZUMA-1 group. While this connection did not appear in the ZUMA-7 data, it underscores the importance of continued investigation into how a patient's inherited genome shapes the biological impact of engineered immune-cell treatments.
This research represents a shift toward more precise medical approaches. By understanding these genetic correlations, clinicians may eventually improve patient risk assessment, refine toxicity monitoring, and inform the future design of clinical trials for cancer immunotherapy.
