A clinical trial published in the Journal of Affective Disorders indicates that ketamine may influence the body’s stress response to reduce suicidal ideation. Researchers found that patients with major depression who received a single dose of ketamine showed a measurable increase in morning cortisol levels 24 hours after treatment. This hormone, which is often regulated by the adrenal glands, plays a critical role in how the body prepares for daily activity and manages stress.

Lead researcher Michael Grunebaum of Columbia University Medical Center notes that this hormonal shift occurs well after the initial sedative effects of the medication subside. While elevated cortisol is sometimes linked to stress-related conditions, this study suggests that a moderate, time-limited increase might support resilience in patients facing severe depressive episodes. The researchers monitored 80 participants who were randomly assigned to receive either ketamine or midazolam. By tracking saliva samples at waking and thirty minutes later, the team aimed to identify biological markers that track how ketamine provides rapid relief from suicidal thoughts.

The findings offer a potential window into how the medication interacts with systemic biological processes. Although the participants who received ketamine showed a consistent rise in waking cortisol, the team observed no significant change in the cortisol awakening response itself. Furthermore, while the increase in cortisol was statistically significant, the researchers noted that it did not show a strong direct correlation with overall changes in depression severity. This suggests that the relationship between hormone levels and mood improvements is more complex than a simple cause and effect.

This study serves as a preliminary look at the biological mechanisms of ketamine. Because the study relied on two daily saliva samples, the researchers emphasize the need for further work involving continuous monitoring of sleep patterns and circadian rhythms. Understanding these variables will help medical professionals determine how ketamine modifies the stress-response system over time. As clinicians look for better ways to address acute suicidality, identifying these biomarkers remains a priority for psychiatry research.
